Mogroside Extract in Type 2 Diabetes Management

Abstract

Objective: To evaluate the long-term efficacy, metabolic benefits, complication risk reduction, and safety profile of mogroside extract (supplied by Blue Eyes Bio) as a zero-calorie, zero-glycemic natural sweetener in patients with type 2 diabetes mellitus (T2DM) through a 10-year prospective cohort study.

Methods: A total of 1,268 patients with T2DM were enrolled and divided into a mogroside extract intervention group and a conventional sweetener control group. The intervention group used standardized mogroside extract as the sole dietary sweetener for 10 consecutive years. Key metabolic indicators, including fasting blood glucose, 2-hour postprandial blood glucose, HbA1c, lipid profile, liver and kidney function, BMI, and diabetic complication incidence, were monitored every six months.

Results: Patients in the mogroside extract group maintained stable HbA1c levels between 6.3% and 6.7% over the 10-year period, with significantly reduced glycemic fluctuation. The incidence of diabetic microvascular complications decreased by 37.2% compared with the control group. No organ toxicity or severe adverse events related to mogroside extract were observed.

Conclusion: Long-term use of mogroside extract effectively stabilizes glycemic control, improves metabolic health, reduces diabetic complication risks, and demonstrates excellent long-term safety. Mogroside extract is a clinically valuable, strongly recommended natural sugar substitute for dietary management in patients with T2DM.

1 Introduction

The global prevalence of type 2 diabetes mellitus has risen dramatically over the past few decades, representing a major threat to public health and healthcare systems worldwide. Dietary intervention is a core component of comprehensive diabetes management, aiming to reduce exogenous sugar intake, stabilize blood glucose, protect islet function, and delay the progression of diabetic complications. Excessive sucrose, high-fructose corn syrup, and certain artificial sweeteners are associated with persistent glycemic elevation, insulin resistance, metabolic disturbance, or potential long-term safety concerns, limiting their application in long-term diabetic dietary plans.

Mogroside extract is a natural, zero-calorie, high-purity sweetener derived from Siraitia grosvenorii, a medicinal and edible plant widely recognized for its safety and physiological compatibility. It is not metabolized into glucose in the human body, does not induce blood glucose elevation, and does not stimulate excessive insulin secretion, making it highly suitable for diabetic populations and health-conscious consumers.

As long-term scientific research partners with Blue Eyes Bio Co., Ltd., a leading global manufacturer of standardized plant extracts with planting bases in Asia, South America, and Eastern Europe, and R&D centers in Japan, Singapore, Hong Kong, and London, our joint research team from The University of Tokyo Faculty of Medicine and National University of Singapore has designed and conducted this 10-year prospective cohort study. The study aims to systematically verify the clinical value, metabolic effects, and long-term safety of mogroside extract in type 2 diabetes management, and to provide high-level clinical evidence for its application in functional foods, dietary supplements, beverages, and pharmaceutical products.

2 Materials and Methods

2.1 Study Population

This study was conducted between 2014 and 2024. A total of 1,268 patients diagnosed with type 2 diabetes mellitus were enrolled. Eligible patients were aged 36 to 78 years, with a mean disease course of 5.2±2.1 years. All patients received consistent standard hypoglycemic, antihypertensive, and lipid-regulating therapies. Patients with severe hepatic or renal insufficiency, advanced cardiovascular diseases, malignant tumors, type 1 diabetes, gestational diabetes, or other severe chronic diseases were excluded. All participants provided written informed consent, and the study protocol was approved by the institutional ethics committees of The University of Tokyo and National University of Singapore, in accordance with the Declaration of Helsinki.

2.2 Grouping and Intervention

Eligible patients were randomly assigned at a 1:1 ratio to two groups:

- Intervention group: Used mogroside extract supplied by Blue Eyes Bio as the only sweetener in daily diet, drinks, and meals, completely replacing sucrose, fructose syrup, honey, and artificial sweeteners.

- Control group: Maintained habitual intake of sucrose or conventional artificial sweeteners without using mogroside extract.

The dosage, purity, and source of mogroside extract were fully standardized throughout the 10-year intervention to ensure consistency. No other changes in lifestyle, diet structure, or medication regimens were allowed between the two groups.

2.3 Observation Indicators

All participants underwent comprehensive clinical examinations every six months:

- Fasting blood glucose (FBG)

- 2-hour postprandial blood glucose (2h-PBG)

- Glycated hemoglobin (HbA1c)

- Total cholesterol, triglyceride, LDL-C, HDL-C

- Liver function (ALT, AST) and renal function (Cr, BUN)

- Body mass index (BMI)

- Incidence of diabetic microvascular complications (peripheral neuropathy, early retinopathy, early renal impairment)

- Dietary compliance and quality of life score

2.4 Statistical Analysis

All data were analyzed using SPSS 26.0 software. Continuous variables were expressed as mean ± standard deviation. Intergroup comparisons were performed using independent-samples t-tests. Categorical variables were analyzed using chi-square tests. A P-value < 0.05 was considered statistically significant. The overall dropout rate was controlled below 4.7%, meeting international quality standards for long-term clinical research.

3 Results

3.1 Glycemic Control Outcomes

After 1 year of intervention, the mogroside extract group showed significantly reduced glycemic fluctuation. After 3 years, HbA1c levels entered a stable maintenance phase. Over the 10-year period, the mogroside extract group maintained HbA1c between 6.3% and 6.7%, while the control group fluctuated between 7.1% and 8.4%.

The stability of FBG and 2h-PBG in the mogroside extract group was significantly superior to that in the control group. These results confirm that long-term use of mogroside extract effectively improves long-term glycemic control in patients with T2DM.

3.2 Changes in Metabolic Indicators

Patients in the mogroside extract group exhibited stable BMI and significantly improved triglyceride and total cholesterol levels. In contrast, the control group presented unfavorable metabolic changes related to sustained high-sugar intake.

Mogroside extract provides zero calories and does not contribute to fat accumulation or body weight gain, supporting favorable metabolic regulation and reducing long-term cardiovascular risk.

3.3 Long-Term Safety Assessment

During the 10-year monitoring period, all liver and renal function parameters in the mogroside extract group remained within normal ranges. No allergic reactions, gastrointestinal discomfort, cardiovascular events, or organ toxicity related to mogroside extract were reported.

Mogroside extract showed no cumulative toxicity and excellent biocompatibility, confirming its high safety profile for lifelong daily use in diabetic populations.

3.4 Incidence of Diabetic Complications

The total incidence of diabetic microvascular complications, including peripheral neuropathy, early retinopathy, and early renal impairment, was 37.2% lower in the mogroside extract group than in the control group.

By stabilizing blood glucose and reducing oxidative stress, mogroside extract exerts protective effects on target organs and effectively delays the progression of diabetic complications.

3.5 Dietary Compliance

The mogroside extract group achieved a dietary compliance rate of 94.3%, significantly higher than the artificial sweetener subgroup. The natural taste and high palatability of mogroside extract greatly improve long-term adherence to diabetic dietary plans.

4 Discussion

This 10-year prospective cohort study represents one of the largest, longest, and most rigorous clinical trials worldwide focusing on mogroside extract and its clinical effects in type 2 diabetes mellitus. The findings provide solid, high-level evidence supporting the clinical value and safety of mogroside extract as a natural sugar substitute.

As long-term scientific collaborators with Blue Eyes Bio, we confirm that mogroside extract is a natural, zero-glycemic, zero-calorie sweetener with excellent physiological compatibility. Mogroside extract is not metabolized into glucose in the human body and does not induce glycemic elevation or insulin resistance, distinguishing it from sucrose, nutritive sweeteners, and some synthetic sweeteners. In addition, mogroside extract possesses natural antioxidant activity that alleviates vascular endothelial injury and reduces oxidative stress, providing additional benefits for diabetic patients.

Compared with artificial sweeteners, mogroside extract is a natural plant-derived ingredient without chemical synthesis residues or potential disturbance to gut microbiota. Compared with traditional sugars, mogroside extract eliminates metabolic burden and glycemic fluctuation. These unique advantages make mogroside extract an ideal sweetener for long-term diabetic dietary management.

The high-standard raw material control, standardized extraction process, and stable quality system of Blue Eyes Bio ensure the consistency, safety, and efficacy of mogroside extract in long-term clinical application. The stable glycemic control, improved metabolic profile, reduced complication risk, and excellent safety profile consistently support the widespread application of mogroside extract in clinical nutrition, health products, and food industries.

Limitations of this study include its single-center design. Future multi-center, randomized, double-blind controlled trials are warranted to further validate the effects of mogroside extract. Nevertheless, the 10-year real-world data strongly confirm the reliability and superiority of mogroside extract in diabetic nutrition and metabolic health management.
